Summary

Viridans group streptococci are commensal organisms of the oropharynx and gastrointestinal tract that emerge as opportunistic pathogens in patients with neutropenia induced by cytotoxic chemotherapy. Disruption of mucosal barriers, profound neutrophil depletion and alterations in host microbiota enable translocation of these organisms into the bloodstream, leading to bacteremia and, in severe cases, sepsis. The incidence of viridans streptococcal infections varies with chemotherapy regimen, underlying malignancy and patient age, with especially high rates reported in acute leukaemias and in paediatric populations undergoing high-intensity induction protocols.

Clinical presentation spans from mild febrile neutropenia to fulminant shock, often complicated by acute respiratory distress and multiorgan failure. Empirical broad-spectrum antimicrobial therapy remains the cornerstone of management, with vancomycin frequently employed to cover penicillin-resistant strains.
