Summary
Visceral adiposity refers to the accumulation of fat within the abdominal cavity, surrounding internal organs, and is a recognised driver of cardiometabolic dysfunction. Unlike subcutaneous fat, visceral fat exhibits heightened lipolytic activity and secretes pro-inflammatory adipokines that impair insulin sensitivity, elevate blood pressure and dysregulate lipid profiles. Research over the past decade has elucidated the mechanistic links between visceral adipose tissue and the pathogenesis of metabolic syndrome, type 2 diabetes and cardiovascular disease. Interventions targeting visceral fat reduction span lifestyle modification, pharmacotherapy and emerging device-based approaches. Dietary strategies emphasise macronutrient composition and caloric restriction, while structured exercise regimens exploit enhanced fatty acid oxidation in visceral depots. Pharmacological agents such as glucagon-like peptide 1 receptor agonists have demonstrated selective reductions in visceral adiposity alongside improvements in glycaemic control. Novel imaging and biochemical indices now permit earlier detection of visceral adipose dysfunction, enabling personalised intervention and dynamic monitoring of treatment response. Collectively, these advances underscore the global imperative to integrate precise assessment of visceral fat into cardiometabolic risk stratification and to refine interventions that specifically attenuate visceral adiposity.

Recent investigations have refined quantitative measures of visceral fat and demonstrated their prognostic value for metabolic outcomes. A longitudinal cohort study of nearly 2 000 apparently healthy adults employed serial imaging to show that increases in visceral adipose tissue area were robustly associated with incident metabolic syndrome over five years, whereas expansion of subcutaneous depots correlated inversely with risk factors such as elevated blood pressure and dysglycaemia. Separately, a novel composite index integrating age, anthropometry and routine lipid parameters was developed and validated for the prediction of type 2 diabetes and pre-diabetes. This index outperformed traditional measures—including body mass index and waist circumference—by achieving higher discrimination of future dysglycaemia in community-based populations. These findings establish precise phenotyping of visceral adiposity as a keystone for early identification of individuals at high cardiometabolic risk and for tailoring preventive strategies.
