Summary

Vitamin D is a fat-soluble secosteroid that undergoes sequential hydroxylation in the liver and kidney to yield the active hormone 1α,25-dihydroxyvitamin D. This metabolite binds the vitamin D receptor (VDR) to regulate intestinal calcium and phosphate absorption, bone mineralisation and parathyroid hormone secretion. Beyond skeletal homeostasis, vitamin D influences immune cell differentiation, inflammatory pathways, cardiovascular function and cellular proliferation. Circulating 25-hydroxyvitamin D is the chief biomarker of status and reflects cutaneous synthesis, dietary intake and genetic determinants. Deficiency manifests classically as rickets in children and osteomalacia in adults, but suboptimal levels have also been linked to osteoporosis, cardiovascular disease, insulin resistance, autoimmune disorders and certain cancers. Genetic studies, experimental models and clinical observations have elucidated the interplay between environmental factors, epigenetic programming and receptor-mediated signalling in shaping systemic health outcomes. Global public-health strategies centre on sensible ultraviolet exposure, dietary recommendations and targeted supplementation to optimise vitamin D status and mitigate disease risk.

Studies have demonstrated that embryonic vitamin D deficiency epigenetically reprogrammes haematopoietic stem cells via suppression of Jarid2 and activation of the Mef2/PGC1α axis, leading to macrophage-driven insulin resistance in offspring even when vitamin D sufficiency is restored postnatally. Large-scale genome-wide association investigations have expanded the catalogue of loci influencing serum 25-hydroxyvitamin D levels, revealing novel genes involved in lipid and lipoprotein metabolism, dermal synthesis pathways and phase II conjugation processes. In parallel, Mendelian randomisation analyses embedded in these population data suggest that various physiological traits and disease states exert causal effects on vitamin D status, rather than vice versa, refining our understanding of directionality in epidemiological associations.
