Summary

Vitamin D, a secosteroid acquired through skin synthesis and dietary intake, plays a pivotal role in immune regulation and lung development in early life. Adequate status of its primary circulating form, 25-hydroxyvitamin D (25(OH)D), in pregnant women and young children has been linked to a lower incidence of asthma, recurrent wheezing and other allergic manifestations. Epidemiological studies indicate that maternal deficiency during gestation may disrupt foetal lung morphogenesis and skew neonatal immune responses towards a pro-allergic phenotype. Postnatal supplementation strategies aim to restore vitamin D sufficiency during critical windows of immune maturation, with the goal of enhancing antimicrobial peptide production, regulating T-cell balance and modulating airway microbiota. While observational data frequently report inverse associations between 25(OH)D levels and allergic outcomes, interventional trials have yielded inconsistent results, reflecting variations in baseline status, timing of administration and dosing regimens. Mechanistic research suggests that vitamin D influences gene expression in pathways governing surfactant production, anti-inflammatory cytokine release and sphingolipid metabolism—processes integral to airway resilience. Globally, vitamin D supplementation represents a low-cost, scalable measure with potential to reduce the burden of childhood allergic disease, though optimal protocols remain to be defined through rigorously powered, long-term studies.
