Summary

Xylitol has emerged as a prominent sugar substitute with proven benefits in the prevention and management of dental caries and periodontal disease. As a five-carbon sugar alcohol, xylitol is metabolically inert to acidogenic oral bacteria, thereby reducing plaque acidity and inhibiting growth of Streptococcus mutans. Integration of xylitol into chewing gums, toothpastes and lozenges not only promotes salivary stimulation and enamel remineralisation but also disrupts biofilm formation. Recent formulations combine xylitol with natural anti-inflammatory agents, low-level fluorides or novel prebiotics to amplify therapeutic outcomes. Clinical and preclinical studies demonstrate dose-dependent reductions in caries incidence, gingival bleeding and microbial virulence factors. Deployment of xylitol interventions has significant global relevance, offering a cost-effective, safe adjunct to conventional fluoride programmes and expanding preventive strategies in both paediatric and adult populations.

A multicentre, double-blind trial evaluated a toothpaste containing extra-virgin olive oil, xylitol and betaine in patients with gingivitis. Over a four-month period, users exhibited significant reductions in supragingival biofilm accumulation and gingival bleeding compared with both placebo and a commercial control. Additional benefits included increased non-stimulated salivary flow and a rise in salivary pH, suggesting enhanced buffering capacity. These findings underscore the synergistic potential of combining xylitol with anti-inflammatory and osmoprotective excipients in daily oral care regimens.
