Summary

Alcohol consumption exerts complex effects on the supporting structures of the teeth, with heavy and chronic use linked to an increased prevalence and severity of periodontitis. Ethanol and its metabolic byproducts impair local host defence by altering neutrophil function, promoting oxidative stress and enhancing pro-inflammatory cytokine release in gingival tissues. Concurrently, alcohol-induced shifts in the oral microbiome favour the overgrowth of periodontal pathogens, while reducing beneficial commensals. These changes contribute to deeper periodontal pockets, loss of clinical attachment and accelerated alveolar bone resorption. Patterns of intake—such as binge drinking versus moderate daily use—further influence disease trajectories, with cumulative dose and frequency emerging as important modifiers. Interactions with smoking, nutritional status and systemic conditions amplify risk, whereas abstinence and improved oral hygiene can partially reverse microbial dysbiosis and mitigate bone loss. Given global ageing and rising alcohol use in many regions, understanding these mechanisms is crucial for targeted prevention and clinical management of periodontal disease.
