Summary
Alexithymia denotes a constellation of cognitive-affective traits characterised by difficulty in identifying and describing one’s own emotions, a prevailing externally oriented thinking style and limited imaginative capacity. Assessment traditionally relies on self-report instruments such as the Toronto Alexithymia Scale, while newer multidimensional questionnaires capture both cognitive and affective facets. Emotion processing in alexithymia spans three interacting levels: recognition of emotional cues, internal representation of affective states and regulation of emotional responses. Neuroimaging and behavioural studies reveal that individuals with elevated alexithymia show reduced activation in limbic and prefrontal networks during emotional appraisal, altered interoceptive signalling associated with bodily awareness and a predisposition towards suppressive rather than reappraisal regulation strategies. These features contribute to impaired social communication, increased vulnerability to affective disorders and somatic symptom presentations. Contemporary research seeks to refine assessment through facet-specific measures, to elucidate neurobiological substrates of each processing stage and to develop targeted interventions that bolster emotion identification and adaptive regulation. Advances in digital phenotyping and non-invasive neuromodulation hold promise for personalised approaches to alexithymia, with potential benefits for mental health, pain management and psychosomatic care.
