Allergen immunotherapy (AIT) represents the only disease-modifying treatment for IgE-mediated respiratory allergies, offering long-term symptomatic relief by inducing immunological tolerance. Subcutaneous immunotherapy (SCIT) typically requires clinic visits over several years, while sublingual immunotherapy (SLIT) can be self-administered at home but may suffer from inconsistent dose intake. Poor compliance not only undermines clinical benefit but also reduces quality of life and inflates healthcare costs through wasted doses and repeated symptom management. Conversely, high adherence correlates with sustained reduction in symptom scores, decreased medication use and improved health-related quality of life. Recent work has explored predictive models for non-adherence, protocol modifications to simplify build-up schedules, and support programmes to engage patients. Global studies highlight substantial variations in persistence across regions and age groups, underlining the need for tailored interventions. Beyond efficacy, the ecological impact of immunotherapy—particularly patient travel to clinics—has also emerged as a factor in treatment choice. Overall, optimisation of adherence through technological, educational and logistical strategies is central to maximising the clinical and societal benefits of AIT worldwide.

Recent studies have quantified the environmental and logistical advantages of SLIT over SCIT. In a national Swedish analysis, models combining clinic-location data and transport patterns estimated that switching all patients from SCIT injections to SLIT tablets could cut annual travel-related CO2 emissions by approximately 1,300 tonnes and reduce patient travel time by over 210,000 hours. These findings underscore how route selection influences not only individual adherence through convenience but also broader sustainability goals, suggesting that environmental metrics can inform shared decision-making in AIT.
