Summary

The anterior cruciate ligament (ACL) is a critical stabiliser of the knee joint, and its injury can lead to functional instability, impaired quality of life and an elevated risk of long-term osteoarthritis. Management strategies range from conservative rehabilitation programmes aimed at restoring strength and neuromuscular control to surgical reconstruction for those with persistent instability or high functional demands. Surgical techniques have evolved from non-anatomical approaches to anatomical single- and double-bundle reconstructions, utilising autografts (such as hamstring or patellar tendon) or allografts. Key considerations in reconstruction include graft choice, tunnel placement, fixation method and timing of surgery. Postoperative rehabilitation is staged to facilitate graft incorporation, progressing through early range-of-motion exercises to strength conditioning and sport-specific training. Emerging methods such as biologically augmented repair, combined anterolateral ligament reconstruction and portable biomechanical assessments seek to optimise joint kinematics, accelerate recovery and reduce re-injury rates. The ultimate goal is to restore physiological joint stability, enable safe return to activity and mitigate the development of post-traumatic osteoarthritis.

Technical terms

Anterior cruciate ligament (ACL): A band of dense connective tissue inside the knee that limits anterior translation and rotation of the tibia relative to the femur.

Autograft: Tissue harvested from the patient (e.g. hamstring or patellar tendon) used to reconstruct the ACL.

Allograft: Donor tissue, typically tendon, used for ACL reconstruction when autograft is contraindicated or declined.

Limb Symmetry Index (LSI): Ratio, expressed as a percentage, comparing strength or functional performance of the operated limb to the contralateral limb.

Portable sensing: Use of wearable or camera-based devices outside laboratory settings to measure biomechanical parameters such as joint angles and ground reaction forces.

Ligamentisation: Biological process through which a tendon graft remodels and acquires structural and mechanical properties similar to the native ACL.

Growth differentiation factor 8 (GDF8; myostatin): A cytokine that inhibits muscle growth; its blockade promotes muscle regeneration and may protect against post-injury joint degeneration.

Knee Injury and Osteoarthritis Outcome Score (KOOS): A five-domain patient-reported instrument assessing pain, symptoms, activities of daily living, sport/recreation function and knee-related quality of life.
