Summary

Sports medicine is an interdisciplinary field dedicated to the promotion of physical activity, the prevention and management of injury, and the optimisation of performance and wellbeing in active individuals. It spans pre-participation assessment, emergency care at sporting events, diagnostic evaluation of acute trauma and overuse conditions, and long-term rehabilitation and return-to-play planning. Practitioners integrate knowledge from orthopaedics, general practice, emergency medicine, physiotherapy, exercise physiology and nutrition, working within multidisciplinary teams that may include athletic trainers, physiotherapists, sports psychologists and dietitians. Beyond musculoskeletal issues, sports medicine also addresses cardiovascular and metabolic health, concussion management, paediatric and geriatric considerations, adaptive sport populations and the careful use of pharmacological or regenerative therapies. A comprehensive multi-omic study in a rodent model has mapped the temporal molecular responses to eight weeks of endurance training across blood, plasma and 18 solid tissues. By integrating transcriptomic, proteomic, metabolomic and epigenomic data, the work revealed coordinated regulation of immune, mitochondrial and stress-response pathways, with distinct patterns in muscle, liver and vasculature. This resource offers new insights into the systemic adaptations that underpin the health benefits of regular aerobic exercise.

Analysis of accelerometer data in over 90 000 adults has uncovered that while moderate-to-vigorous physical activity (MVPA) at any time of day correlates with reduced cardiovascular and all-cause mortality, accumulating the bulk of MVPA in the midday–afternoon window confers the greatest protective effect. These findings highlight not only the volume and intensity but also the timing of activity as a modifiable factor in cardiovascular prevention strategies.

Pre-participation evaluation: A comprehensive medical and musculoskeletal assessment performed before engaging in organised physical activity to identify risks and optimise safety.

Neuromuscular control: The coordinated activation of muscles and joints to maintain stability and efficient movement, fundamental in injury prevention and rehabilitation.

Metabolic equivalent (MET): A unit estimating the energy cost of physical activities relative to resting expenditure, facilitating the classification of exercise intensity.
