Summary

Antimicrobial photodynamic therapy (aPDT) represents a minimally invasive adjunctive approach for the management of periodontal diseases, which are driven by complex biofilm communities and host inflammatory responses. The technique combines a photosensitising agent with light of a specific wavelength to generate reactive oxygen species that selectively inactivate pathogenic bacteria within periodontal pockets. Unlike systemic antibiotics, aPDT offers localised microbial control without promoting resistance and can penetrate deep gingival tissues when applied with optimised light sources such as light-emitting diodes or low-power lasers. Clinical outcomes include reductions in probing pocket depth and bleeding on probing, together with improvements in clinical attachment levels and periodontal tissue repair. Additionally, advances in photosensitiser delivery, light-wave modulation and real-time tissue oxygenation monitoring have enhanced the precision and efficacy of aPDT, underlining its potential as a global, cost-effective tool in periodontal therapy.

Recent studies have demonstrated that non-invasive photodynamic therapy can be effectively delivered through an optimised topical formulation of toluidine blue O and irradiated by a 650 nm light-emitting diode to target periodontal pathogens. In vitro experiments confirmed that the light penetrates approximately 3 mm of soft tissue and activates the photosensitiser to achieve significant bactericidal effects against key species such as Porphyromonas gingivalis and Fusobacterium nucleatum. In a rodent model of periodontitis, repeated weekly applications over four weeks markedly reduced alveolar bone loss and down-regulated inflammatory cytokine expression, highlighting the method’s translational promise for safe, adjunctive periodontal care.
