Summary
Attachment theory has emerged as a pivotal framework for understanding how early relational experiences shape vulnerability to psychotic disorders. Secure attachment supports adaptive stress regulation, social cognition and resilience, whereas insecure patterns—anxious, avoidant or disorganised—predispose individuals to dysregulated affect, aberrant threat processing and maladaptive interpersonal schemas. Disorganised attachment, often rooted in trauma or unpredictable caregiving, is particularly implicated in the emergence and severity of hallucinations, delusions and paranoia. Across the psychosis continuum, attachment insecurity is linked to heightened hypothalamic–pituitary–adrenal axis reactivity, dopaminergic dysfunction and impairments in mentalization—the capacity to interpret behaviour in terms of mental states. These mechanisms intertwine to influence symptom expression, help-seeking behaviour and treatment engagement. Interventions targeting attachment dynamics—such as mentalization-based approaches, secure-base priming and tailored cognitive-behavioural strategies—show promise in enhancing social functioning and mitigating distressing psychotic experiences. A global research effort continues to refine assessment tools, clarify neurobiological pathways and integrate attachment-informed staging models to personalise prevention and therapy.
