Summary
Attentional biases in anxiety disorders refer to the preferential allocation of attentional resources to threat-related stimuli, observed across subtypes such as social anxiety disorder, panic disorder and generalised anxiety disorder. These biases manifest at multiple processing stages, including rapid automatic orienting towards perceived threat, prolonged maintenance of attention on threatening cues, and strategic avoidance or disengagement. Experimental paradigms reveal that heightened amygdala reactivity and altered fronto-parietal network dynamics underlie these processes, reflecting an imbalance between bottom-up threat detection and top-down cognitive control. Consistent findings demonstrate that individuals with anxiety exhibit both faster detection of threat-related stimuli and difficulty disengaging from them, contributing to the persistence of anxious states and exacerbation of worry. The robustness of these effects has been shown across behavioural measures and neurophysiological indices, underscoring their transdiagnostic relevance. Clinically, attentional biases serve as potential markers for risk, treatment response and relapse prevention. Emerging interventions seek to recalibrate attentional patterns through cognitive training and pharmacological modulation, with a view to restoring adaptive information-processing and reducing symptomatic distress.
