Cardiovascular Risk and Neurodegenerative Disorders in Parkinson's Disease
Summary
Parkinson’s disease (PD) is increasingly recognised not only as a movement disorder but also as a condition with substantial cardiovascular comorbidity. Epidemiological and mechanistic studies have revealed that patients with PD face an elevated risk of ischaemic stroke, coronary artery disease and other vascular events. Underlying mechanisms include cardiac autonomic dysfunction arising from neurodegeneration of brainstem centres, systemic inflammation driving atherosclerotic changes, and shared genetic predispositions. These interactions underscore the need for integrated screening and preventative measures. Early identification of high-risk individuals may enable tailored interventions—from lifestyle modification to pharmacological prophylaxis—and has global significance given ageing populations and the growing burden of both neurodegenerative and cardiovascular disease.

A Mendelian randomisation analysis using genome-wide association data has established a likely causal link between Parkinson’s disease and increased risk of coronary artery disease and stroke. By selecting genetic variants associated with PD as instrumental variables and applying an inverse variance weighted approach, investigators demonstrated modest but significant odds ratios for coronary artery disease (approximately 1.06) and for stroke subtypes including ischaemic and cardioembolic stroke (approximately 1.04–1.08). Sensitivity analyses, including weighted median and MR-Egger methods, affirmed the robustness of these findings and supported the concept that genetic liability to PD contributes to vascular risk.
