Summary

Cocoa flavanols are a group of polyphenolic compounds predominantly represented by (−)-epicatechin and catechin, abundant in unprocessed cocoa beans and dark chocolate. These bioactives exert multiple actions relevant to cardiometabolic health, including enhancement of endothelial function, modulation of blood pressure, improvement of glucose metabolism and favourable effects on lipid profiles. At the vascular level, cocoa flavanols promote nitric oxide–mediated vasodilation, reduce arterial stiffness and attenuate low-grade inflammation. Through antioxidant and anti-inflammatory mechanisms they may protect against atherogenesis and insulin resistance. Interactions with the gut microbiome generate secondary metabolites with unique bioactivities, contributing to interindividual variability in response. Dietary intake of cocoa flavanols is thus emerging as a promising component of nutritional strategies aimed at prevention and maintenance of cardiovascular and metabolic health at the population level.

Foundational human studies have elucidated the absorption, distribution, metabolism and excretion profile of (−)-epicatechin. Over 80 % of ingested (−)-epicatechin is absorbed and converted into at least 20 distinct metabolites, many of which arise through microbial biotransformation. Species-specific differences in metabolism have highlighted the need for human-based data when translating preclinical findings. A comprehensive pharmacokinetic characterisation revealed key biomarkers for objective assessment of intake and paved the way for dose-response investigations as well as safety evaluations of cocoa flavanol consumption.

Technical terms

Cocoa flavanols: Polyphenolic compounds in cocoa, mainly (−)-epicatechin and catechin, with vascular and metabolic bioactivities.

(−)-Epicatechin: A monomeric flavanol that acts on endothelial nitric oxide synthesis and undergoes extensive metabolism in humans.

Endothelial function: The capacity of blood vessel lining to regulate vascular tone and maintain hemostatic balance, often assessed by vasodilatory response.

Flow-mediated vasodilation (FMD): A non-invasive ultrasound measure of endothelium-dependent dilation in response to increased shear stress.

Cardiometabolic health: A composite concept encompassing cardiovascular and metabolic risk factors such as blood pressure, lipid profile and glucose homeostasis.
