Summary

Adolescence is characterised by biological shifts in circadian timing, heightened psychosocial demands and evolving sleep needs, rendering young people particularly vulnerable to insomnia and other sleep disturbances. Cognitive behavioural interventions address these challenges through structured strategies that modify maladaptive thoughts and behaviours around sleep. Core components include psychoeducation on sleep physiology, stimulus control to reinforce the bed–sleep association, sleep restriction to consolidate rest, cognitive restructuring to challenge unhelpful beliefs about sleep, and relaxation training to reduce physiological arousal. Delivery formats range from individual and group workshops to digital platforms, enhancing accessibility and engagement. Evidence indicates that tailoring these interventions to developmental and cultural contexts improves adherence and outcomes. Importantly, improved sleep during adolescence not only enhances cognitive performance, emotional regulation and school attendance but also mitigates the risk of mood and anxiety disorders, underlining the global public-health significance of early, targeted behavioural treatment.

Longitudinal cohort data have revealed that insomnia beginning in childhood often persists into adolescence and is associated with diminished health-related quality of life and increased incidence of new medical and psychiatric conditions. Persistent sleep disturbance predicted greater need for both general and psychiatric medication by mid-adolescence, even after accounting for socioeconomic factors and comorbid sleep-disordered breathing. These findings underscore the necessity of early behavioural intervention and suggest that effective cognitive behavioural approaches in younger cohorts may yield substantial long-term health benefits.

Technical terms

Cognitive behavioural therapy for insomnia (CBT-I): A structured, multi-component behavioural and cognitive programme designed to alleviate insomnia by targeting unhelpful sleep-related thoughts and habits through psychoeducation, stimulus control, sleep restriction and cognitive restructuring.

Sleep efficiency: The ratio of total sleep time to time spent in bed, expressed as a percentage, used as an objective indicator of sleep quality and consolidation.

Stimulus control: A behavioural strategy within CBT-I aimed at strengthening the association between bed and sleep by limiting non-sleep activities in bed and enforcing a consistent sleep–wake schedule.

Sleep diary: A daily self-monitoring tool in which individuals record bedtime, wake time, awakenings and perceived sleep quality to guide treatment planning and assess progress.
