Summary

Obsessive-compulsive disorder (OCD) is a chronic psychiatric condition marked by intrusive thoughts (obsessions) and repetitive behaviours or mental acts (compulsions). Cognitive behavioural interventions constitute the first-line psychological treatment, combining exposure and response prevention (ERP) with cognitive techniques to challenge maladaptive beliefs. ERP requires systematic confrontation with feared stimuli while refraining from ritualised responses, thereby promoting habituation and inhibitory learning. Cognitive components target dysfunctional appraisals such as overestimation of threat, inflated responsibility and intolerance of uncertainty, using strategies of cognitive restructuring and reality testing. Delivery formats have diversified beyond individual face-to-face therapy to include group sessions, intensive brief formats and therapist-guided or self-guided internet platforms, widening access and reducing dropout rates. Across diverse cultural and healthcare settings, these modalities demonstrate consistent medium to large effect sizes, with durable gains at follow-up. Adjunctive approaches, such as inference-based modalities and digital applications, aim to enhance tolerability and engagement, particularly for patients reluctant to undertake exposure exercises. The global significance of improving delivery and adherence is underscored by the high burden of untreated OCD and its impact on functioning, mortality risk and comorbid conditions such as depression and anxiety. Ongoing research seeks to refine mechanisms of change, personalise intervention components and extend dissemination through telehealth and mobile health solutions.
