Summary
Depression is characterised by pervasive alterations in the way emotional information is perceived, interpreted and regulated. Individuals with depression commonly exhibit a negative attentional bias, selectively attending to threatening or sad stimuli and interpreting ambiguous cues in a pessimistic manner. At a neural level, hyperactivity of the amygdala in response to negative faces and diminished prefrontal cortical control contribute to an impaired ability to reappraise or inhibit negative affect. Reinforcement learning mechanisms are also disrupted, with reduced sensitivity to positive feedback and a tendency to overweigh negative outcomes, compounding anhedonia and motivational deficits. These cognitive–emotional disturbances emerge early in the course of illness and may predict response to treatment. Understanding the interplay between biased information processing, underlying neural circuitry and therapeutic interventions has global significance, informing the development of targeted cognitive and pharmacological strategies to ameliorate symptoms and prevent relapse.

Recent studies have shown that several weeks of selective serotonin reuptake inhibitor administration modulate cortical responses to emotional stimuli rather than principally altering amygdala reactivity. In a controlled study of healthy volunteers receiving escitalopram for three to five weeks, functional imaging revealed attenuated activation in frontal and occipital regions during processing of angry and fearful faces, suggesting a top-down mechanism that may underlie clinical improvement. Complementing these findings, computational modelling work has demonstrated that serotonin reuptake inhibition amplifies the influence of induced positive affect on reward learning. Rather than directly boosting mood or reward sensitivity, antidepressants appear to potentiate an interaction between affect and learning, offering a mechanistic account of the delayed emergence of mood elevation observed in patients.
