Continuous EEG Monitoring in Pediatric Critical Care
Summary
Continuous electroencephalography (cEEG) has become an indispensable tool in the management of acutely ill children with neurological compromise. By providing uninterrupted recordings of cortical electrical activity, cEEG enables the timely detection of both convulsive and non-convulsive seizures, informs prognostic assessments, and guides antiepileptic therapy. In paediatric intensive care units (PICUs), where sedated or comatose patients may exhibit subtle electrographic events, continuous monitoring offers a window into evolving brain function and helps stratify risk of adverse outcomes. Advances in equipment miniaturisation, automated pattern recognition and wireless platforms are widening access to prolonged neuromonitoring, even beyond tertiary centres. Despite logistical challenges such as electrode placement time, data interpretation demand and resource constraints, recent work has demonstrated practical strategies to optimise montage configurations, integrate amplitude-integrated displays and deploy low-cost wearable systems. As a result, cEEG is increasingly regarded not only as a diagnostic adjunct but also as a core component of individualised, brain-targeted critical care.

Recent studies have evaluated the trade-off between practicality and diagnostic yield in cEEG electrode configurations. One investigation assessed seizure detection using a reduced eight-lead montage in a paediatric epilepsy monitoring setting. Compared with the full 21-electrode array, the simplified montage maintained high specificity but exhibited a modest reduction in sensitivity for focal and regional seizure patterns. These findings support tailored electrode strategies in resource-limited environments, balancing rapid application against slight decreases in detection of certain seizure foci.
