Cryotherapy and Recovery Modalities in Athletic Performance

Summary

Athletic performance is intimately linked to effective recovery strategies that mitigate fatigue, inflammation and muscle damage. Cryotherapy, encompassing techniques such as cold‐water immersion, whole‐body cryotherapy and topical cold packs, aims to lower tissue temperature, constrict blood vessels and reduce metabolic demand in exercised muscles. Complementary modalities include contrast water therapy, heat application, compression garments and active recovery. These interventions are employed to accelerate tissue perfusion, attenuate delayed‐onset muscle soreness and restore neuromuscular function. While mechanistic studies highlight anti‐inflammatory and analgesic effects, consensus on optimal temperature, duration and combination protocols remains under development. Current investigations seek to balance the benefits of acute cooling against potential impairments in muscle adaptation, with an emphasis on tailoring recovery to sport‐specific demands and individual responses.

Recent studies have compared the immediate effects of cold, heat and alternating compression therapies on muscle recovery in combat sports athletes. One trial reported that heat and contrast protocols enhanced tissue perfusion and elasticity more effectively than cold alone, while cold therapy increased muscle tone without compromising strength. Another investigation assessed repeated cold‐water immersions following exercise-induced muscle damage, finding that multiple immersions attenuated muscle swelling and markers of structural damage, yet did not modify systemic inflammatory cytokine profiles or accelerate functional strength recovery over a week. Together, these findings underscore the selective benefits of cryotherapy on biomechanics and local tissue responses, while highlighting limited systemic anti-inflammatory impact.

Contrast therapy: Alternating use of cold and heat to provoke vasoconstriction and vasodilation cycles, enhancing circulation.

Delayed-onset muscle soreness (DOMS): Post-exercise pain and stiffness arising hours to days after unfamiliar or strenuous activity.

Neuromuscular fatigue: Decline in muscle force or power generation due to peripheral or central factors during or after exercise.
