Dietary Influences on Chronic Kidney Disease Management

Summary

Chronic kidney disease (CKD) affects multiple metabolic pathways, and nutritional interventions are critical to slow progression, reduce complications and improve quality of life. Dietary modulation targets key pathogenic processes including glomerular hyperfiltration, acid–base imbalance, mineral dysregulation and inflammation. Protein restriction within recommended ranges alleviates glomerular pressure and mitigates uraemic toxin accumulation. Adjustment of sodium and phosphate intake supports blood pressure control and mineral bone health, while optimisation of energy intake prevents protein–energy wasting. More recently, emphasis has shifted towards overall dietary patterns rather than individual nutrients. Plant-centric diets, Mediterranean-style regimens adapted for renal function and enhanced fibre intake have been shown to favourably modulate gut microbiota, reduce oxidative stress and improve metabolic profiles. Nutritional strategies must be individualised for CKD stage, comorbidities and cultural context, bearing in mind global disparities in food availability and diet-related disease burden.

Estimated glomerular filtration rate (eGFR): A calculated index of kidney function based on serum creatinine, age and sex, used to stage CKD.

Glomerular hyperfiltration: An initial increase in filtration rate per nephron that can accelerate kidney damage when sustained.

Net endogenous acid production: The net amount of acid generated by metabolism of dietary nutrients, influencing acid–base homeostasis.

Dietary fibre: Non-digestible carbohydrates from plant sources that modulate gastrointestinal transit, microbial fermentation and systemic metabolism.
