Summary
Diuretic therapy remains a cornerstone in the management of heart failure, targeting intravascular and interstitial volume overload to alleviate symptoms, improve exercise tolerance and reduce hospital admissions. Loop diuretics achieve potent natriuresis by inhibiting the Na+-K+-2Cl– cotransporter in the thick ascending limb of the loop of Henle, while thiazides, mineralocorticoid antagonists and carbonic anhydrase inhibitors provide adjunctive mechanisms to address residual fluid retention and neurohormonal activation. Despite clear symptomatic benefits, variability in patient response, risks of electrolyte disturbances, renal impairment and diuretic resistance pose ongoing clinical challenges. Optimising dose regimens, timing of administration and combination strategies has become essential to balance decongestion with preservation of renal function and avoidance of adverse events. While this approach improved early decongestion metrics, it did not translate into reductions in mortality or rehospitalisation at six months, underscoring the need for further refinement of personalised protocols. Mechanistic insights into renal handling of sodium and the interplay between tubular function and systemic haemodynamics are informing the next generation of diuretic dosing algorithms aimed at maximising decongestive efficacy while minimising renal and electrolyte complications.
