Summary

The vascular endothelium is a dynamic monolayer of specialised cells lining the inner surface of blood and lymphatic vessels. Far from serving as a passive barrier, endothelial cells actively regulate vascular tone through the synthesis of vasodilators and vasoconstrictors, maintain haemostatic balance by orchestrating coagulation and fibrinolysis, and control the selective exchange of fluid, solutes and macromolecules. They guide immune cell trafficking and mount local inflammatory responses, while also directing angiogenesis during tissue repair. Endothelial heterogeneity—manifest as organ-specific gene expression, structural adaptations such as fenestrations or continuous junctions, and distinct receptor profiles—underpins the specialised functions of vessels in different tissues. Vascular health relies on the integrity of endothelial mechanotransduction pathways, intercellular junctions and the glycocalyx, with dysfunction leading to hypertension, atherosclerosis, thrombosis and chronic inflammatory disorders. Understanding these fundamental processes has global significance for the prevention and treatment of cardiovascular and inflammatory diseases.

Recent studies have revealed organ-specific inflammatory responses in endothelial cells. Investigations across vascular beds from heart, lung, kidney and liver have demonstrated distinct patterns of adhesion molecule upregulation and chemokine secretion that shape tissue-specific immune cell recruitment. Differences in baseline immune-regulating gene expression appear to underpin variable vascular inflammation and rejection risk in organ transplantation. Furthermore, analysis of human male and female endothelial cells has uncovered intrinsic sexual dimorphism in endothelial Nitric Oxide Synthase expression and activity. Female cells exhibit higher eNOS levels and nitric oxide production, which in turn modulate migratory and sprouting behaviours during angiogenesis. Male cells rely more on proliferation for capillary outgrowth, suggesting that sex-specific signalling should inform therapeutic strategies for cardiovascular disease.
