Summary

Erythropoietin is a key regulator of erythroid progenitor proliferation and differentiation, widely used to correct anaemia in chronic kidney disease and oncology settings. Beyond its haematopoietic role, erythropoietin influences vascular tone and systemic blood pressure through multiple pathways. Acutely, erythropoietin receptor activation in endothelial cells enhances nitric oxide synthesis, promoting vasodilatation. Chronically, elevated erythropoietin doses increase red blood cell mass and blood viscosity, augment vascular resistance and modulate vascular remodelling. In clinical practice, sustained erythropoietin administration is associated with a dose-dependent rise in systolic and diastolic pressure, attributable to altered autonomic regulation, heightened sensitivity to vasoconstrictors and augmented reactive oxygen species production. Understanding these mechanisms is critical for balancing the correction of anaemia against potential hypertensive complications and for guiding therapeutic strategies that optimise cardiovascular outcomes worldwide.
