Summary
Accumulating evidence demonstrates that regular physical exercise exerts robust anxiolytic and antidepressant effects across diverse populations. Structured aerobic and resistance training protocols modulate central and peripheral systems implicated in mood regulation. Mechanisms include upregulation of neurotrophic factors such as brain-derived neurotrophic factor (BDNF), enhancement of synaptic plasticity, attenuation of neuroinflammation via reduction in pro-inflammatory cytokines, and recalibration of the hypothalamic–pituitary–adrenal (HPA) axis. Exercise influences monoaminergic transmission by increasing availability of serotonin, norepinephrine and dopamine in key limbic structures, and promotes hippocampal neurogenesis, thereby improving emotion processing and cognitive resilience. Functional neuroimaging studies reveal that sustained activity programmes strengthen connectivity within prefrontal–limbic circuits, which underlie regulation of stress and affective states. Clinically, both high-intensity interval training and moderate continuous exercise have been shown to reduce symptom severity in individuals with diagnosed depression or anxiety disorders, offering a cost-effective, low-risk adjunct to conventional therapies. Global health initiatives emphasise implementation of tailored exercise prescriptions, recognising individual variability in response and adherence. Further research aims to refine intensity, duration and modality parameters to maximise therapeutic outcomes.
