Exercise Interventions for Anxiety Disorders
Summary
Exercise interventions encompass structured programmes of physical activity that are increasingly recognised as effective adjunctive treatments for anxiety disorders. Epidemiological studies have established a robust inverse association between regular physical activity and anxiety prevalence, suggesting both preventive and therapeutic benefits. Aerobic modalities, such as running or cycling, alongside resistance training, have been shown to modulate neurochemical systems—including the hypothalamic–pituitary–adrenal axis, brain-derived neurotrophic factor pathways and monoaminergic transmission—leading to anxiolytic effects. Moreover, mechanistic research points to the regulation of neural circuits implicated in threat processing, notably the amygdala and prefrontal networks, as critical substrates for symptom reduction. Clinical trials have demonstrated that a minimum threshold of exercise frequency, intensity and duration correlates with clinically meaningful improvements, while meta-analytic data support a dose–response relationship favouring moderate-to-vigorous regimens. Importantly, exercise interventions also address comorbid physical health risks endemic to anxiety disorders, such as cardiovascular morbidity, thereby offering a dual therapeutic strategy. Tailoring exercise prescriptions to individual symptom profiles and physical capacities, and integrating behaviour-change techniques to enhance adherence, are key priorities for translating research into practice.

In parallel, neuroimaging research has elucidated the rapid neural adaptations following acute exercise, showing that habitual physical activity levels mediate changes in amygdala reactivity to emotional stimuli and alter functional connectivity with prefrontal and limbic regions. These findings underscore both the psychological and neurobiological mechanisms through which exercise exerts anxiolytic effects and inform the development of optimised intervention protocols.

Dose–response relationship: A correlation between the amount (dose) of an intervention, such as exercise intensity or duration, and the magnitude of the observed therapeutic effect.
