Exercise Interventions in Non-Alcoholic Fatty Liver Disease

Summary

Non-Alcoholic Fatty Liver Disease (NAFLD) encompasses a spectrum from simple steatosis to steatohepatitis and fibrosis, representing a major public health concern worldwide. Lifestyle modification remains the first-line therapy, with exercise forming a cornerstone of non-pharmacological management. Aerobic modalities, resistance training and high-intensity interval training (HIIT) have been studied for their capacity to reduce hepatic lipid content, improve insulin sensitivity and attenuate visceral adiposity independent of weight loss. Mechanistic insights point to enhanced mitochondrial function in muscle, modulation of adipose tissue gene expression and alterations in systemic inflammation. Emerging evidence suggests that personalised approaches—tailoring exercise dose, intensity and combination with dietary strategies—may optimise outcomes. Across diverse populations, consistent engagement in moderate-to-vigorous activity of at least 150 minutes per week has demonstrated clinically meaningful reductions in liver fat, while HIIT protocols offer time-efficient alternatives. The global burden of NAFLD and its cardiometabolic sequelae underscores the imperative for pragmatic exercise prescriptions integrated within multidisciplinary care.

Recent studies have probed the interaction between exercise, diet and host biology to refine treatment of steatotic liver disease. A 2022 randomised controlled trial in patients with NAFLD and prediabetes revealed that combined aerobic exercise and fibre-enriched diet stabilised keystone gut microbial taxa and enhanced network connectivity. Baseline microbiome profiles predicted individual reductions in liver fat, pointing towards microbiome-guided personalisation of exercise interventions. Earlier seminal work compared resistance training, high-intensity interval aerobic training and moderate-intensity continuous training over 12 weeks in sedentary obese men with NAFLD. While all regimens achieved similar reductions in hepatic fat, only high-intensity interval training improved liver stiffness and restored Kupffer cell phagocytic function through modulation of inflammation and fatty acid metabolism.
