Gingival Blood Flow Dynamics in Periodontal Health and Disease
Summary
Gingival blood flow is governed by a dynamic microvascular network that ensures tissue perfusion, oxygen delivery and waste removal. In health, tight regulation of capillary tone and vessel architecture maintains homeostasis, supports immune surveillance and facilitates wound healing. Endothelial cells and perivascular cells coordinate vasomotor responses through nitric oxide, prostaglandins and inflammatory mediators to adjust perfusion to local demands. In periodontal disease, persistent microbial challenge and host immune activation disrupt this balance. Increased vascular permeability, pathological angiogenesis and altered vessel morphology lead to microcirculatory dysfunction, impaired oxygenation and accumulation of pro-inflammatory cytokines. Changes in capillary diameter, flow velocity and microvessel density accompany gingival inflammation and progressive attachment loss, while systemic risk factors such as age, sex and comorbid conditions further modulate vascular responses. Advances in optical imaging and in vivo flow measurement have deepened understanding of these processes and informed novel diagnostic and therapeutic strategies, including physical stimulation techniques, targeted anti-inflammatory treatments and personalised oral hygiene interventions.
