Glycemic Index and Load Effects on Metabolic Health
Summary
The glycaemic index (GI) and glycaemic load (GL) provide complementary measures of carbohydrate quality, quantifying both the speed and magnitude of postprandial blood glucose rises. Diets characterised by lower GI and GL values have been associated with improved glycaemic control, enhanced insulin sensitivity and reductions in long-term markers such as haemoglobin A₁c. In mechanistic studies, low-GI/GL patterns attenuate rapid glucose excursions, modulate appetite through effects on satiety hormones and may favourably influence lipid profiles. At a population level, high dietary GI and GL have been implicated in the pathogenesis of type 2 diabetes, obesity and cardiovascular disease, with observed dose–response relations in diverse ethnic and geographical settings. Public health strategies that integrate GI and GL with measures of dietary fibre and whole-grain intake are emerging as pragmatic approaches to lower chronic disease risk. However, the relative contributions of total carbohydrate load, glycaemic response and broader dietary patterns continue to be refined through ongoing clinical trials and observational analyses, underscoring the need for tools that translate GI/GL concepts into accessible guidance for healthcare professionals and the public alike.
