Summary

Hamstring flexibility plays a critical role in the mechanics of the lumbopelvic region and the development, persistence and management of low back pain. Tight or shortened hamstrings limit hip flexion, forcing compensatory increases in lumbar flexion and anterior pelvic tilt during activities such as bending or lifting. This altered lumbo-pelvic rhythm elevates intervertebral disc pressure and overstresses posterior elements of the spine, contributing to pain generation and recurrence. Across diverse populations—from sedentary office workers to athletes and manual labourers—restricted hamstring extensibility has emerged as a modifiable musculoskeletal risk factor for the onset of low back pain and as a determinant of functional disability once pain ensues. Assessment of hamstring length using clinical tests informs both prevention strategies and targeted rehabilitation programmes. Interventions that restore hamstring extensibility may normalise spinal biomechanics, reduce pain intensity and improve functional performance, highlighting the global significance of this research for ergonomic guidelines, physiotherapy practice and public health initiatives.

Technical terms

Hamstring flexibility: the capacity of the hamstring muscle group to lengthen and allow full range of hip flexion with the knee extended.

Straight Leg Raise (SLR) test: a clinical measure of hamstring extensibility in which the patient lies supine and the examiner lifts the straightened leg, recording hip flexion angle at the onset of resistance or pain.

Active Knee Extension (AKE) test: an assessment of hamstring length performed with the hip flexed at 90° while the patient actively extends the knee, noting the maximal extension angle.

Lumbo-pelvic rhythm: the coordinated movement between the lumbar spine and the pelvis during trunk flexion or extension.

Lumbar lordosis: the inward curvature of the lumbar spine, which influences load distribution and spinal flexibility during movement.
