Health Impacts of Religious Fasting and Dietary Patterns
Summary
Religious fasting and associated dietary regimens represent a complex interplay of cultural practice, temporal restriction of food intake and specific nutrient prescriptions. From the daily dawn-to-dusk abstention of Ramadan to the periodic vegan-style observances of Christian Orthodox and Ethiopian Orthodox traditions, these regimens combine elements of intermittent energy restriction, time-restricted feeding and plant-centred nutrition. Collectively, they have been linked to reductions in body weight, improvements in body composition, enhancements in lipid and glucose regulation, and modulation of inflammatory markers. Mechanistic studies highlight shifts in metabolic pathways, including increased ketogenesis and altered lipoprotein profiles, while observational and interventional work points to benefits for cardiometabolic health and cancer risk modulation. The global reach of these practices—encompassing over a billion adherents to Islam and millions of Orthodox Christians—confers both population-level significance and practical avenues for non-pharmacological intervention. Emerging evidence underlines the necessity of balancing nutrient adequacy with potential micronutrient gaps and tailoring recommendations according to individual health status and cultural context.
Research from Nature Portfolio
A longitudinal cohort study of Ethiopian Orthodox fasting evaluated the combined effect of energy restriction, time-restricted feeding and vegan dietary patterns during two major fasting seasons. Using bioelectrical impedance analysis to assess body composition before and after 44-day and 14-day fasting periods, researchers observed significant decreases in total body weight, fat-free mass and trunk fat mass, even after adjustment for age, sex and physical activity. Longer fasting durations yielded proportionately greater reductions. These findings suggest that structured religious fasting may serve as an effective non-pharmacological strategy for prevention or management of obesity and related chronic conditions.

Technical terms
Time-restricted feeding: A regimen limiting daily food intake to a specified window of hours, often aligned with circadian rhythms.
Bioelectrical impedance analysis (BIA): A non-invasive technique that estimates body composition by measuring electrical conductivity through bodily tissues.
Metabolomics: The large-scale study of small-molecule metabolites within biological samples, offering insight into metabolic pathway changes.
Lipoprotein subclasses: Distinct categories of lipid-transporting particles (for example, VLDL, LDL, HDL) that differ in size, density and atherogenic potential.
C-reactive protein (CRP): An acute-phase protein produced by the liver, commonly used as a biomarker of systemic inflammation.
