Heart Failure Dynamics and Therapeutic Interventions
Summary
Heart failure represents a clinical syndrome in which structural or functional impairment of ventricular filling or ejection leads to symptoms of breathlessness, fatigue and fluid retention. Central to its pathophysiology is the interplay between haemodynamic alterations, neurohormonal activation and maladaptive remodelling. A decline in left ventricular ejection fraction can result from myocyte loss, fibrosis and adverse chamber dilation, whereas preserved ejection fraction phenotypes often reflect diastolic stiffness, endothelial dysfunction and microvascular inflammation. Activation of the renin–angiotensin–aldosterone system and sympathetic nervous system initially supports cardiac output but chronically drives fluid overload, vasoconstriction and further myocardial injury. Counter-regulatory pathways, including natriuretic peptides and nitric oxide–cyclic GMP signalling, are overwhelmed as disease advances. Therapeutic interventions aim to rebalance these systems by inhibiting angiotensin-converting enzyme, blocking angiotensin receptors, antagonising mineralocorticoid receptors and deploying angiotensin receptor–neprilysin inhibitors. Additional strategies encompass sodium–glucose cotransporter 2 inhibitors, cardiac resynchronisation, ventricular assist devices and tailored lifestyle modifications. Early diagnosis and timely initiation of multidrug regimens improve symptom control, reduce hospital admissions and extend survival. Emerging approaches seek to refine patient selection through biomarkers, imaging and genomic profiling, thereby ushering an era of precision medicine in heart failure management.
Research from Nature Portfolio
Recent work has highlighted the cardiovascular benefits of angiotensin receptor–based therapy in patients with concomitant renal dysfunction. In cohorts with markedly reduced glomerular filtration and high albuminuria, angiotensin receptor blockade halved the incidence of composite cardiovascular events, including hospitalisation for heart failure, compared with non-blockade regimens. This effect was achieved without compromising renal filtration or raising natriuretic peptide levels, underscoring the capacity of targeted neurohormonal modulation to interrupt the vicious cycle of cardiorenal deterioration.
Heart Failure Dynamics and Therapeutic Interventions publication trend
The graph below shows the total number of articles in heart failure dynamics and therapeutic interventions across all publications each year (not limited to Nature Index journals).
Technical terms
Ejection fraction: Proportion of blood ejected from the left ventricle with each heartbeat, expressed as a percentage.
Renin–angiotensin–aldosterone system (RAAS): Hormonal cascade regulating blood pressure, fluid balance and vascular tone that becomes maladaptive in chronic heart failure.
Angiotensin receptor–neprilysin inhibitor (ARNI): Combined therapy that blocks angiotensin receptors while inhibiting neprilysin to enhance natriuretic peptide activity.
Neprilysin: Zinc-dependent enzyme responsible for degradation of natriuretic peptides, bradykinin and other vasoactive peptides.
Neurofilament light chain (NfL): Plasma biomarker of neuronal injury, elevated in various neurological and systemic conditions.
