Heart Failure Epidemiology and Clinical Management Strategies

Heart failure represents a growing global health challenge marked by rising prevalence, considerable morbidity and mortality, and substantial healthcare expenditure. Population ageing and improved survival after acute cardiovascular events have driven an increase in both incidence and absolute numbers of people living with heart failure. Socioeconomic disparities influence age at onset, comorbidity burden and outcomes, with deprived groups experiencing earlier onset and worse prognosis. Clinical management has evolved from a one-size-fits-all paradigm to a patient-centred, multidisciplinary approach combining guideline-directed medical therapy, device implantation, remote monitoring and structured education. Early identification of left ventricular systolic dysfunction, optimisation of neurohormonal blockade and tailored interventions for comorbidities such as diabetes, renal impairment and atrial fibrillation underpin contemporary strategies. Integration of biomarker assessment, digital health tools and specialised heart failure clinics supports more precise risk stratification, treatment intensification and coordinated follow-up across primary and secondary care settings.

Recent studies have evaluated systematic echocardiographic screening in high-risk pacemaker recipients to detect left ventricular systolic dysfunction at an earlier stage. In a multicentre trial, nearly one-third of asymptomatic individuals were found to have previously unrecognised systolic impairment; subsequent management in a specialist heart failure and devices clinic appeared to reduce adverse outcomes compared with standard primary care follow-up. These findings underscore the potential of structured imaging pathways linked to specialist services to refine early detection and optimise therapeutic adjustment, although screening per se did not alter event rates without integrated care.

Technical terms

Left ventricular systolic dysfunction (LVSD): Impairment of the left ventricle’s ability to contract and eject blood, often assessed by a reduced ejection fraction.

Ejection fraction (EF): The proportion of blood pumped out of the left ventricle with each heartbeat, expressed as a percentage; a key indicator of systolic function.

Guideline-directed medical therapy (GDMT): Evidence-based pharmacological regimens (including ACE inhibitors, beta-blockers, mineralocorticoid receptor antagonists and angiotensin receptor–neprilysin inhibitors) recommended to improve survival and reduce hospital admissions in heart failure.

Multimorbidity: The coexistence of two or more chronic conditions (such as diabetes, renal impairment or atrial fibrillation) that complicates management and influences prognosis in heart failure patients.
