Immunological Strategies Against Porphyromonas gingivalis-Induced Periodontal Disease

Summary

Porphyromonas gingivalis is recognised as a keystone pathogen in chronic periodontitis, driving a dysbiotic shift in the subgingival microbiome and promoting destructive inflammation. Immunological strategies against P. gingivalis centre on both modulation of innate defences and induction of pathogen-specific adaptive responses. Innate approaches include targeting pattern-recognition receptors on epithelial cells and dendritic cells to recalibrate inflammatory signalling and preserve tissue homeostasis. Adaptive strategies focus on eliciting protective antibodies against key virulence factors such as gingipains, fimbrial proteins and capsular polysaccharides. Vaccine platforms under investigation range from subunit constructs with recombinant antigens and built-in adjuvants to conjugate formulations that enhance T-cell help and high-affinity memory. Mucosal immunisation via nasal or oral routes aims to induce secretory IgA in saliva and gingival crevicular fluid, thereby preventing bacterial colonisation and biofilm formation. Collectively, these immunological interventions hold promise as adjuncts to mechanical debridement and antimicrobial therapy, with the goal of preventing alveolar bone loss and preserving periodontal health.
