Summary

Physical activity has emerged as a modifiable factor that can influence susceptibility to infection, disease severity and recovery in the context of COVID-19. Regular engagement in moderate to vigorous physical activity supports immune surveillance, enhances vaccine responsiveness and attenuates chronic inflammation, thereby reducing the risk of severe respiratory illness. Improved cardiorespiratory fitness and metabolic health also mitigate the prevalence and impact of comorbid conditions such as obesity, hypertension and type 2 diabetes, which are known to exacerbate COVID-19 complications. Exercise induces the release of myokines and other biomolecules that modulate systemic immune function and maintain endothelial integrity, counteracting the endothelial inflammation and cytokine dysregulation characteristic of critical COVID-19. From community-based programmes to home- and outdoor-based regimens, incorporating tailored physical activity into public-health strategies has the potential to strengthen population resilience, lower rates of hospitalisation and shorten convalescence. Ongoing research is refining optimal exercise prescriptions, characterising dose-response relationships and exploring differential effects across age groups, sexes and clinical risk profiles.
