Summary

Smoking remains one of the most significant modifiable risk factors for periodontal disease worldwide. Combustible tobacco products introduce numerous toxins, including nicotine and free radicals, that compromise the vascular supply to gingival tissues and disrupt immune cell function. The resultant microvascular constriction diminishes oxygenation and nutrient delivery, while alterations in neutrophil and macrophage activity impede the clearance of pathogenic biofilms. Smokers not only experience deeper periodontal pockets and greater attachment loss but also demonstrate reduced clinical signs of inflammation, masking disease progression. At the molecular level, tobacco constituents inhibit fibroblast proliferation and collagen synthesis, leading to accelerated alveolar bone resorption. Crucially, smoking attenuates the response to both non-surgical and surgical periodontal therapies, necessitating tailored treatment protocols. Globally, the burden of smoking-related periodontal disease underpins substantial morbidity, reinforcing the need for integrated cessation strategies within dental practice.

Microvascular dysfunction: Impaired function or structure of small blood vessels, leading to reduced tissue perfusion and compromised healing.

Cytokine: A signalling protein released by immune cells that modulates inflammation and tissue repair in periodontal tissues.
