Insomnia Treatment Strategies and Sleep Mechanisms

Summary

Insomnia affects a substantial proportion of the global population and arises from a complex interplay of neurobiological, endocrine and behavioural factors. Traditional approaches encompass cognitive behavioural therapy for insomnia (CBT-I), which targets maladaptive sleep habits and dysfunctional beliefs, alongside pharmacological agents such as hypnotics that modulate GABAergic or melatonergic pathways. Advances in understanding sleep homeostasis and circadian regulation have highlighted the roles of the hypothalamic–pituitary–adrenal (HPA) axis, noradrenergic circuits centred on the locus coeruleus and inhibitory GABAergic neurons in the ventrolateral preoptic area (VLPO). Emerging modalities include neuromodulation, next-generation orexin receptor antagonists and integrative strategies involving dietary supplements or functional foods. Converging evidence from transcriptomics, metabolomics and neuroimaging is revealing how synaptic plasticity, glymphatic clearance and neuroinflammatory processes contribute to both sleep initiation and maintenance. These insights promise more precise, mechanism-based interventions to restore healthy sleep architecture and improve quality of life.

A foundational study has elucidated how chronic elevation of corticosterone can precipitate insomnia-like disturbances by downregulating glucocorticoid receptors in the locus coeruleus, thereby over-activating noradrenergic output and inhibiting GABAergic neurons in the VLPO. Local antagonism of these receptors was found to reverse sleep fragmentation and extend non-rapid eye movement sleep, underscoring the pivotal role of stress-responsive molecular pathways in sleep regulation. This work offers a mechanistic framework for developing targeted agents that modulate receptor signalling within defined brainstem circuits.

GABAergic signalling: Inhibitory neurotransmission mediated by γ-aminobutyric acid acting primarily through GABAA and GABAB receptors to promote neuronal hyperpolarisation and sleep induction.

Non-rapid eye movement (NREM) sleep: A restorative phase of sleep characterised by slow-wave brain activity, reduced muscle tone and synchronised neuronal firing.

Hypothalamic–pituitary–adrenal (HPA) axis: A neuroendocrine system involving the hypothalamus, pituitary gland and adrenal cortex that regulates stress responses and influences sleep–wake cycles.

Locus coeruleus: A brainstem nucleus rich in noradrenergic neurons that modulates arousal and vigilance via widespread projections to cortical and subcortical structures.

Ventrolateral preoptic area (VLPO): A hypothalamic region containing sleep-promoting GABAergic neurons that inhibit wake-active centres to facilitate sleep initiation.

Sleep latency: The interval between lights-out and the onset of sustained sleep, often used as an objective measure of insomnia severity.
