Internet-Delivered Cognitive Behavioral Therapy for Anxiety Disorders in Youth
Summary
Internet-delivered cognitive behavioural therapy (ICBT) has emerged as a scalable, evidence-based intervention to address anxiety disorders in children and adolescents. By translating core CBT techniques—such as cognitive restructuring, exposure exercises and skills training—into self-paced online modules, ICBT programmes extend reach beyond traditional clinics and reduce barriers of cost, geography and stigma. Models range from wholly self-guided, open-access platforms to multicomponent treatments incorporating real-time therapist support via telephone or videoconferencing. Stepped-care frameworks further optimise resources by offering low-intensity interventions first and “stepping up” to guided support only when symptoms persist. Meta-analyses and large-scale implementation studies demonstrate moderate to large reductions in anxiety symptoms, with stronger outcomes typically seen in programmes that integrate human guidance. Key challenges include maintaining engagement, predicting and preventing dropout, and ensuring consistent therapeutic alliance in a digital environment. Advances in persuasive design, automated monitoring and adaptive pacing are addressing these limitations, while cost-effectiveness analyses underscore the potential for health-system savings and increased school attendance. Globally, ICBT is now being integrated into stepped-care pathways and school-based prevention efforts, highlighting its role as both a first-line treatment and a preventive tool in youth mental health.
