Intolerance of Uncertainty in Anxiety Disorders
Summary
Intolerance of uncertainty (IU) describes a dispositional incapacity to endure the unknown, manifesting as heightened distress, worry and behavioural avoidance when faced with ambiguity. Originally characterised in relation to generalized anxiety disorder, IU is now recognised as a transdiagnostic mechanism contributing to a spectrum of anxiety conditions, including social anxiety, panic disorder and obsessive–compulsive disorder. Individuals high in IU tend to overestimate threat likelihood, engage in repetitive negative thinking and show inflexible coping responses, all of which amplify and prolong anxious arousal. Neurocognitive studies link IU to altered functioning in prefrontal and limbic circuits that govern emotion regulation and cognitive control. Across cultural contexts, IU predicts greater symptom severity and poorer treatment response unless directly targeted. Clinically, interventions that reduce intolerance of uncertainty—through exposure to uncertainty, cognitive restructuring of catastrophic beliefs and training in decision-making under ambiguity—have yielded promising outcomes. The global significance of IU lies in its role in shaping resilience or vulnerability to stressors ranging from developmental transitions to large-scale crises, highlighting the need for early assessment and tailored interventions to mitigate anxiety-related impairment.

Recent longitudinal work has clarified how IU drives anxiety by disrupting emotion regulation and interacting with cognitive control. A multi-wave study tracked changes in IU, emotion-regulation difficulties and anxiety symptoms during a global health crisis. Results demonstrated that increases in IU predicted subsequent rises in stress and depression through deteriorating emotion regulation skills. Crucially, individuals with stronger cognitive flexibility and executive control showed a weaker link between IU and regulation difficulties, suggesting these capacities buffer against uncertainty-induced distress. This insight points to the value of enhancing cognitive control as a protective target in interventions aimed at strengthening resilience under uncertainty.
