Lactate monitoring has become integral to the management of critically ill patients, serving as both a marker of metabolic stress and a guide for therapeutic intervention. Elevated blood lactate concentrations often reflect an imbalance between oxygen delivery and cellular demand, but they may also arise from increased glycolytic flux driven by catecholamine surge. Trends in lactate levels—rather than a single measurement—have demonstrated robust prognostic value across diverse acute conditions, including sepsis, trauma and cardiogenic shock. Serial assessment can inform the adequacy of resuscitation, signal evolving organ dysfunction and refine risk stratification beyond conventional scoring systems. Advances in point-of-care testing have enabled rapid bedside measurements, facilitating dynamic titration of fluids, vasopressors and ventilatory support. Globally, standardised lactate-guided protocols have been adopted to improve outcomes, reduce intensive care unit (ICU) length of stay and optimise resource allocation in high-volume settings.

Recent studies have demonstrated that integrating lactate measurements with established organ-failure scores enhances mortality prediction in sepsis. A large prospective cohort evaluated a combined lactate-score and Sequential Organ Failure Assessment (SOFA) framework over the first three days of ICU admission. The novel composite score yielded a consistent increase in area under the receiver operating characteristic curve for in-hospital mortality—from moderate discriminative power at baseline to substantially higher accuracy by day three—outperforming the SOFA score alone at each time point. These findings underscore the value of serial lactate assessment as a dynamic adjunct to conventional severity scoring, enabling earlier identification of patients at highest risk and informing targeted escalation of therapy.
