Summary

Aortic coarctation repair in infancy or childhood has evolved into a highly successful intervention, yet survivors face lifelong cardiovascular sequelae. Even after anatomically satisfactory correction, up to one-third of patients develop systemic hypertension, reflecting persistent vascular dysfunction. The repair site and adjacent aortic segments often exhibit reduced distensibility and increased stiffness, contributing to elevated pulse wave velocity and abnormal wave reflections that impose a chronic pressure load on the left ventricle. Over time this afterload may lead to left ventricular hypertrophy, diastolic dysfunction and impaired exercise tolerance. Recoarctation and aneurysm formation at the repair site remain important mechanical complications requiring vigilant imaging surveillance. Cerebrovascular anomalies, including intracranial aneurysms and variations in the circle of Willis, add to the neurovascular risk profile. Early intervention appears to mitigate some adverse remodelling, but even optimally treated individuals require lifelong follow-up with tailored blood pressure control, non-invasive haemodynamic assessment and timely re-intervention when necessary. Emerging insights into arterial biomechanics and ventricular–vascular coupling are guiding refined management strategies to improve quality of life and long-term survival.

Exercise-induced hypertension: An exaggerated systolic blood pressure response during physical activity that exceeds age- and sex-specific thresholds.

Recoarctation: The re-narrowing of the aorta at or near the surgical repair site, which may necessitate further intervention.
