Summary

Maladaptive Schema Theory proposes that a set of deeply rooted cognitive–emotional patterns, known as early maladaptive schemas, develops in response to unmet core emotional needs during childhood. These schemas act as enduring templates through which individuals interpret experiences, regulate affect and form interpersonal expectations. When activated by life stressors, schemas foster biased information processing, amplify emotional reactivity and sustain self-defeating behaviours. The original model describes 18 schemas clustered into domains such as disconnection and rejection, impaired autonomy and performance, impaired limits, other-directedness and over-vigilance and inhibition. Empirical research has demonstrated their transdiagnostic relevance across depression, anxiety, personality disorders, eating problems and addiction. Schema Therapy, an integrative approach drawing on cognitive, behavioural and experiential techniques, aims to modify these entrenched patterns by combining cognitive restructuring, limited reparenting and imagery rescripting. Cross-cultural studies confirm the global applicability of schema constructs, while contemporary work explores neural correlates and dynamic schema networks. Clinically, assessment of schema severity can guide personalised interventions, enhance treatment adherence and predict long-term outcomes in diverse populations.

Computational modelling research has begun to formalise schema updating as a belief-revision process in anxiety, demonstrating that inflexible schema priors lead to perseverative avoidance, while targeted interventions enhance prediction error signalling and cognitive flexibility.
