Management of Acute Aortic Dissection

Acute aortic dissection is a life-threatening condition characterised by the separation of the aortic wall layers, demanding rapid risk stratification, diagnosis and tailored intervention. Initial management centres on haemodynamic stabilization with blood pressure reduction and heart rate control to limit propagation of the intimal tear. Diagnostic pathways combine clinical assessment, imaging modalities such as computed tomography angiography and magnetic resonance imaging, and emerging use of circulating biomarkers to expedite exclusion or confirmation. Definitive therapy varies by dissection type: surgical repair remains the gold standard for proximal (Stanford type A) dissections with open replacement of the ascending aorta, often extended via hybrid techniques such as the frozen elephant trunk; distal (Stanford type B) dissections are managed medically in uncomplicated cases, with thoracic endovascular aortic repair reserved for complications including malperfusion, rupture or refractory pain. Multidisciplinary teams coordinate perioperative intensive care, address organ support and apply evidence-based protocols to reduce mortality. Advances in device technology, procedural planning and risk prediction now inform patient selection, while global registries and quality-improvement initiatives aim to harmonise care and narrow outcome disparities.
