Summary
Matrix metalloproteinases (MMPs) are a family of zinc-dependent proteases that orchestrate extracellular matrix turnover within the periodontium. Under physiological conditions, they facilitate tissue remodelling, wound repair and embryogenesis by cleaving collagen, proteoglycans and other matrix proteins. In periodontal disease, dysregulated MMP expression and activation drives connective tissue breakdown, alveolar bone resorption and gingival inflammation. Elevated levels of MMP-8 (collagenase-2) and MMP-9 (gelatinase-B) are consistently detected in gingival crevicular fluid, saliva and peri-implant sulcular fluid, correlating with clinical parameters of disease severity. The balance between MMPs and their endogenous inhibitors (TIMPs) is crucial for maintaining periodontal health. Advanced diagnostic approaches now focus on quantifying active MMP species in oral fluids, offering real-time, non-invasive assessment of disease activity, early detection of tissue destruction and monitoring of therapeutic outcomes. This paradigm shift from conventional probing and radiography to biomarker-based diagnostics promises to enhance patient stratification and personalised care in periodontal and peri-implant therapy.
