Summary

First responders—including firefighters, police officers, paramedics and other emergency personnel—face frequent exposure to traumatic incidents and high-pressure environments. Such repeated encounters can precipitate a range of mental health outcomes, from acute stress reactions to chronic conditions such as post-traumatic stress disorder (PTSD), depression, anxiety and substance misuse. Occupational stressors including shift work, life-threatening events and organisational demands may accumulate over time, exacerbating risks of burnout and compassion fatigue. Barriers to help-seeking, notably stigma and fears about career repercussions, can delay or deter engagement with psychological support services. Emerging evidence emphasises the protective roles of peer support, resilience training and employer-provided trauma-focused interventions. Attention is increasingly turning to the wider first responder community, including family members, whose well-being is intertwined with that of the responder. Efforts to improve mental health outcomes now encompass multilevel approaches: individual coping strategies, team-based peer support networks and systemic cultural changes promoting open dialogue and accessible care pathways.
