Mind-Body Interventions for Chronic Low Back Pain
Summary
Chronic low back pain (CLBP) is a leading cause of disability worldwide and presents a major socioeconomic and individual burden. Mind-body interventions integrate physical postures, controlled breathing and mental focus to address both physiological function and psychosocial factors. Common approaches include yoga, tai chi, qigong, Pilates and mindfulness-based stress reduction, each aiming to enhance core stability, flexibility and body awareness while reducing stress, muscle tension and pain perception. Mechanistically, these practices may modulate central pain processing, restore autonomic balance and improve self-efficacy in pain management. Delivered in group classes, individual sessions or via digital platforms, mind-body therapies offer nonpharmacological alternatives or complements to conventional care, with applicability across diverse populations. Evidence supports modest to moderate improvements in pain intensity, functional disability and psychological well-being, and highlights the importance of instructor training, intervention dose and long-term adherence for optimal outcomes.
Research from Nature Portfolio
A systematic review and meta-analysis of randomised trials evaluated the effects of tai chi across chronic pain conditions, including low back pain. Data from 18 controlled studies demonstrated significant immediate reductions in pain (standardised mean difference around –0.8) and improvements in function following at least five weeks of practice. Benefits persisted short term, with lower but still meaningful effect sizes at follow-up. The analysis underscored tai chi’s potential as a viable complementary therapy for CLBP, emphasising its safety profile, the importance of consistent practice and the need for standardisation of style and intensity in future trials.
Mind-Body Interventions for Chronic Low Back Pain publication trend
The graph below shows the total number of articles in mind-body interventions for chronic low back pain across all publications each year (not limited to Nature Index journals).
Technical terms
Mind-body intervention: A therapeutic approach that combines physical activity with mental and emotional regulation to influence physiological function and pain perception.
Standardised mean difference (SMD): A statistical measure of effect size expressing the difference between intervention and control groups relative to variability.
Autonomic balance: The equilibrium between sympathetic and parasympathetic nervous system activity, important for stress response and pain modulation.
Craniosacral therapy: A gentle manual technique aiming to optimise cerebrospinal fluid dynamics and relieve tension in the craniosacral system.
Self-efficacy: An individual’s belief in their capacity to organise and execute actions required to manage prospective situations, here in relation to pain.
