Summary

Mindfulness-based interventions (MBIs) encompass structured programmes that cultivate present-moment awareness through formal meditation practices, gentle movement and psychoeducational components. Originating in clinical and contemplative traditions, the two most widely implemented MBIs are Mindfulness-Based Stress Reduction (MBSR) and Mindfulness-Based Cognitive Therapy (MBCT). Both target dysregulated stress responses and maladaptive cognitive patterns by fostering an attitude of non-judgemental observation of thoughts, bodily sensations and emotions. A robust evidence base now supports their efficacy across a range of mental health conditions, including depression, anxiety, chronic pain and addiction. Mechanistic studies suggest that MBIs enhance attentional control, emotion regulation and decentring—the ability to observe internal experiences without overidentification—while also engaging frontoparietal control networks. Digital and self-administered formats have broadened access globally, offering cost-effective stress reduction and symptom management for diverse populations. Collectively, MBIs represent a versatile, transdiagnostic approach with enduring effects on psychological well-being and quality of life.

Recent multi-site data confirm the short-term stress-reducing potential of standalone mindfulness exercises delivered in self-administered digital formats. In a large preregistered study involving over two thousand adult participants, four brief exercises were compared with an active control condition. All mindfulness practices yielded greater reductions in self-reported stress, with the body scan exercise demonstrating the largest effect. Bayesian mixed-effects analyses indicated a moderate effect size across interventions, supporting their scalability for English-speaking, higher-income populations. These findings underscore the feasibility of remote, low-cost delivery and provide a template for integrating digital mindfulness into public health strategies.

Active control: A comparison condition in which participants receive an alternative intervention, allowing researchers to control for non-specific treatment effects.

Body scan: A formal mindfulness exercise in which attention is systematically directed to sensations in different parts of the body to cultivate present-moment awareness.
