Summary

Nightmares, characterised by vivid and distressing dreams that awaken individuals, are a significant component of trauma-related distress. They frequently co-occur with conditions such as post-traumatic stress disorder, mood and anxiety disorders, and may arise in idiopathic forms without clear traumatic antecedents. Chronic nightmares interfere with sleep quality and daytime functioning, perpetuating hyperarousal and impaired emotion regulation. Their aetiology involves dysregulated neural circuits governing fear extinction during REM sleep, maladaptive cognitive and emotional processing of threat memories, and, in some cases, genetic predispositions. Understanding nightmare phenomena is critical for developing targeted interventions and mitigating long-term health risks.

A cross-cultural investigation of dream content has provided fresh insights into the emotional adaptive function of dreams across diverse societies. Forager populations exhibited heightened communal and threat-related dream imagery, yet demonstrated lower negative emotional valence in dream narratives compared to urban groups. These differences were attributed to strong social norms and interpersonal support fostering emotional regulation during sleep. The findings suggest that cultural and environmental factors shape dream processes, enabling non-clinical populations to rehearse threat scenarios within reassuring contexts and thereby reduce overall anxiety. This work underscores the evolutionary significance of dreaming as a mechanism for affective processing.
