Summary

Patellofemoral pain (PFP) is characterised by diffuse anterior knee discomfort arising from the articulation between the patella and femoral trochlea. Evaluation encompasses a combination of patient history, clinical examination of patellar tracking and alignment, assessment of muscle strength and flexibility, and, where indicated, non-invasive imaging or biomechanical modelling. Management prioritises graduated exercise therapy to restore quadriceps and hip musculature balance, optimise movement patterns and modulate joint loading. Adjunctive strategies include patellar taping or bracing to correct maltracking, foot orthoses when biomechanical foot faults are present, and patient education to address pain-related fear and promote adherence. Emerging techniques for evaluation integrate advanced computational modelling with joint reaction force analysis and imaging to quantify patellofemoral joint stress more precisely. Rehabilitation protocols now employ loading indices to categorise exercises by patellofemoral load and guide progressive dosing. A stepwise, multidisciplinary approach ensures that conservative measures are escalated only when exercise alone proves insufficient, thereby reducing unnecessary interventions and healthcare burden.
