Summary

Patient activation and engagement refer to the process by which individuals with long‐term health conditions acquire the knowledge, skills and confidence to actively participate in their own care. High levels of activation are associated with improved self-management behaviours, better adherence to treatment plans and reduced healthcare utilisation. Engagement encompasses collaborative decision‐making, shared goal-setting and sustained interaction between patients and healthcare providers. Recent advances highlight the role of tailored interventions, digital tools and psychosocial support in elevating activation among diverse patient groups. Effective strategies address health literacy, self-efficacy and social determinants of health, recognising that activation is not a fixed trait but a dynamic state that may fluctuate with disease progression, life events and access to resources. Globally, efforts to embed activation metrics within routine care pathways are gaining momentum, enabling the design of precision support for those at risk of poor outcomes. As healthcare systems pivot towards value-based models, patient activation emerges as both a meaningful outcome and a modifiable determinant of quality, equity and cost-effectiveness in chronic disease management.

A cross-sectional analysis demonstrated that patients with higher literacy and stronger support networks exhibit greater confidence in self-management tasks, while those experiencing severe disease flares benefit most from psychosocial interventions. Modelling of activation pathways suggested that tailored education and peer support can shift patients from passive recipients of care to proactive self-managers, with implications for reducing flares and healthcare utilisation.
